Position Overview

As a Patient Care Coordinator, you will play a vital role in our Care Coordination Centre, ensuring seamless communication and support for patients and healthcare professionals.

Employment Details:

This is a permanent role requiring a commitment of 20 hours per week.

Key Responsibilities:

  • Serve as the primary contact for healthcare professionals seeking emergency secondary care.
  • Manage both inbound and outbound communications with patients and medical staff.
  • Facilitate the scheduling of outpatient appointments and hospital admissions.
  • Input patient information accurately into our systems, maintaining confidentiality and integrity.
  • Provide administrative assistance to clinicians and colleagues as needed.

Essential Qualifications:

  • Demonstrated experience in call handling or customer service.
  • Strong communication skills and a professional telephone demeanor.
  • Ability to handle sensitive information with discretion.
  • Proficient in computer usage and data entry.
  • Capable of working independently and collaboratively within a team.

Additional Information:

While prior experience in a healthcare setting is advantageous, it is not mandatory as comprehensive training will be provided.
