Job Overview

The Patient Journey Coordinator position is situated within the Integrated Discharge Hub, where you will play a crucial role in assisting ward-based Patient Journey Facilitators in implementing the SAFER Red2Green strategy, while also supporting the Integrated Discharge Team (IDT) with daily discharge operations.

The SAFER Red2Green initiative is designed to enhance patient experience by minimizing delays throughout their hospital stay.

We are looking for a motivated individual with a proactive mindset who can collaborate effectively with integrated health and social care teams, as well as Patient Journey Facilitators, to ensure a smooth progression of the patient journey.

The ideal candidate will possess strong interpersonal skills and the ability to thrive under pressure, meeting tight deadlines. A solid understanding of the SAFER Red2Green approach and various patient pathways is preferred to ensure timely progression of patients' journeys.

Key Responsibilities

- Communicate updates and pertinent information to ward-based Patient Journey Facilitators and Ward Coordinators to guarantee safe and timely patient discharges.

- Maintain daily records in the database regarding discharge failures across multiple sites.

- Proactively manage patient pathways, ensuring that investigation and treatment schedules are aligned with targets.

- Monitor the progress of each patient on the Complex Worklist, ensuring timely interventions are executed.

- Participate in daily IDT meetings to identify patients needing social services or Clinical Commissioning Group (CCG) support for discharge facilitation.

- Ensure the accuracy of data entered into the Computerised SEMA system and update PSAG Boards with relevant plans and actions.

About Our Organization

The Patient Journey Coordinator role is essential in assisting wards to ensure that patients are discharged safely and efficiently.

This position is part of the Capacity Centre, working alongside Clinical Site Managers to support wards in managing patient journeys and discharge processes.

This dynamic and fast-paced role offers variety, as you will collaborate closely with a range of multidisciplinary teams to ensure that patient pathways and planned discharges are executed smoothly and effectively.

Job Requirements

Qualifications

Essential

  • 5 GCSEs at Grade 9 - 4 or equivalent (including Maths and English)

Desirable

  • Knowledge of Medical Terminology
  • ECDL Certification

Experience

Essential

  • Experience in Data Collection and Validation
  • Extensive background in a healthcare setting

Desirable

  • Familiarity with patient pathways

Skills and Knowledge

  • Proficiency in MS Office Applications
  • Full PC Literacy
  • Knowledge of medical terminology
  • Advanced keyboard proficiency
  • Ability to report and present information effectively
