Position Overview

This role offers a unique opportunity to contribute to the development of our community sub-acute wards, working alongside a dynamic multidisciplinary team.
As a Patient Transition Coordinator, you will play a crucial role in facilitating timely discharges, ensuring that patients can return to their homes or usual places of care as swiftly as possible.
Key Responsibilities
- Collaborate within the ward setting as part of a multidisciplinary team to ensure a seamless and coordinated approach to the discharge of patients with varying needs.
- Engage with patients, their families, and other essential stakeholders to support the discharge process effectively.
- Implement the SAFER red to green methodology to enhance patient care and expedite discharge.
- Exercise a significant degree of autonomy while working within the ward team, taking initiative in your role.
