About This Role
We are seeking a dedicated and experienced Patient Discharge Coordinator to join our team at Shropshire Community Health NHS Trust. In this role, you will play a vital part in ensuring the smooth and timely discharge of patients from our wards, enabling them to return home or to their next place of care as quickly and safely as possible.
Your Responsibilities
- Collaborate closely with multidisciplinary teams within the ward environment to coordinate patient discharges, addressing both simple and complex needs.
- Maintain clear and effective communication with patients, their families, carers, and external stakeholders involved in the discharge process.
- Contribute to the implementation of the SAFER red to green approach, streamlining patient care pathways and expediting discharges.
- Work autonomously within the ward team, demonstrating initiative and sound judgment to ensure timely and efficient patient transitions.
