About the Role

We are seeking a highly skilled and organized Patient Administration Care Coordinator to join our team at Shrewsbury Primary Care Network. The successful candidate will be responsible for ensuring that patients' medical notes are summarized efficiently and accurately, following established protocols and guidelines.

Key Responsibilities
  • Summarize incoming patients' medical records, following an agreed protocol.
  • Process hospital letters, outpatient reports, and results, and transpose relevant information onto patients' clinical records.
  • Code and link each problem appropriately, review and enrich the summary page, and ensure QOF performance markers are captured.
  • Identify gaps or errors in medical records and rectify as necessary, or flag for further attention.
  • Record allergies and sensitivities, and alert the recall clerk to potential recalls.
  • Ensure safeguarding information is flagged and identified.
  • Match electronic records, validate medical history, and tidy the home page.
  • Check medication, allergies, and sensitivities in degraded GP2GP data.
  • Check medical records content for online record access and report findings to the usual GP.

Requirements
  • Demonstrated capabilities to manage own workload and make informed decisions.
  • Experience of office procedures, working at a high level as part of an administration team or within an administration role.
  • Experience of setting up and implementing internal processes and procedures.
  • Experience of dealing with sensitive and confidential information.
  • Proven administrative experience.
Desirable Qualifications and Skills
  • Good standard of education, GCSE or equivalent in English and Mathematics.
  • Level 3 or equivalent experience in admin, business, marketing, or customer service environment.
  • Understanding of Clinical Governance and Confidentiality and Data Protection Act.
  • Working knowledge of database management and local guidelines.
