Patient Administration Care Coordinator

We are seeking a highly skilled and organized Patient Administration Care Coordinator to join our team at Shrewsbury Primary Care Network. The successful candidate will be responsible for ensuring that patients' medical notes are summarized efficiently and accurately, following established protocols and guidelines.
Key Responsibilities- Summarize incoming patients' medical records, following an agreed protocol.
- Process hospital letters, outpatient reports, and results, and transpose relevant information onto patients' clinical records.
- Code and link each problem appropriately, review and enrich the summary page, and review QOF performance markers.
- Identify gaps or errors in medical records and rectify as necessary, or flag these gaps if unable to find any data.
- Record all allergies and sensitivities, and alert the recall clerk to potential recalls that need setting up or reviewing.
- Ensure that information on safeguarding children or vulnerable adults is appropriately flagged and identified.
- Match electronic records on the computer, validate existing medical history, and complete any gaps.
- Check medication, allergies, and sensitivities in degraded GP2GP data.
- Check medical records content for online record access, report findings back to the usual GP, and check medical records for patients choosing to have an enhanced summary care record.
Shrewsbury PCN has a collective list size of patients across Shrewsbury and surrounding areas. Our main aims are to address our wider population health needs and health inequalities, ensure patients have access to a larger range of professional groups to aid health issues, support clinical decision making, make the best use of data and technology available to us, and work together more effectively.
Requirements- Demonstrated capabilities to manage own workload and make informed decisions.
- Experience of office procedures working at a high level as part of an administration team or within an administration role.
- Experience of setting up and implementing internal processes and procedures.
- Experience of dealing with sensitive or confidential information.
- Experience of working in reception or telephone environments.
- Proven administrative experience.
- Good standard of education, GCSE or equivalent in English and Mathematics.
- Ability to use own initiative when appropriate, build and maintain effective working relationships, communicate with a high level of effectiveness both verbally and in writing, and work under pressure with constant interruptions.
- Understanding of Clinical Governance, Confidentiality, and Data Protection Act.
