About the Role
The Shrewsbury and Telford Hospital NHS Trust is seeking a highly skilled Trauma Rehabilitation Coordinator to join our team. As a key member of the multidisciplinary team, you will play a crucial role in ensuring the seamless coordination of care and rehabilitation for patients who have suffered major trauma.
Key Responsibilities
- Provide professional leadership and direction to the multidisciplinary team in the planning and coordination of the trauma patient pathway.
- Work closely with the Emergency Department and Trauma Consultants to coordinate and communicate the rehabilitation and repatriation pathways for patients who have suffered major trauma.
- Manage a live database of patients who have suffered major trauma to track progress and ensure optimal recovery.
- Act as an advocate for patients with trauma-related injuries, coordinating their acute stay, rehabilitation, and discharge from the Trauma Unit.
- Provide clinical leadership to inpatient therapy staff and coordinate the care of trauma patients regardless of their specialty.
- Work collaboratively within the multidisciplinary team to facilitate a smooth and safe transition from hospital to community or on to further rehabilitation.
- Contribute to the investigation and analysis of trauma risks, issues, incidents, and death, and implement learning from actions.
Requirements
- Degree-level qualification or equivalent.
- Professional registration with the Health and Care Professions Council (HCPC) or Nursing and Midwifery Council (NMC).
- Extensive postgraduate clinical experience working within a multidisciplinary team at band 6 or above in a range of relevant clinical areas, including major trauma, trauma orthopaedics, neuro specialties, spinal injuries, and critical care.
- Experience of clinical leadership and ability to work autonomously.
- Excellent communication and organisational skills, with the ability to lead, support, coach, and motivate others.
