About the Role

We are seeking a highly motivated and experienced Mental Health Rehabilitation Support Worker to join our team at Trident Group. The successful candidate will be based in Shrewsbury and will provide rehabilitation-focused support to individuals with complex and enduring mental health needs.

Key Responsibilities

  • Deliver and promote a high standard of support for each individual in the home, including demonstrating and advising on good practice.
  • Promote individual customers' independence and choice.
  • Prompt individual customers with all aspects of personal hygiene and appearance as required by the individual.
  • Advising and assisting in developing a range of social, leisure, work, education or other meaningful activities as appropriate with customers.
  • Support customers in preparing and cooking meals, including menu planning and budgeting.
  • Support customers to take an active role within the community.
  • To support customers in preparations to move on from services, liaising with care coordinators, social workers and other care providers.
  • Establish and maintain links with other healthcare professionals and agencies.
  • Undertake the key-working role as designated by your line manager, including regular key-work sessions, positive outcomes, reviewing support plans and preparing and participating in review meetings alongside the customer.
  • Undertake initial and on-going assessments in consultation with the customer, manager and involved agencies.
  • Promote and uphold the Trident Group policy on Equality and Diversity.
  • Staff must maintain the confidentiality of all customers and any staff information they become aware of.
  • Adhere to the Trident Group Policies & Procedures and Code of Conduct.

About You

We are looking for a highly motivated individual who would like to join our team to deliver good quality service to vulnerable adults. The ideal candidate will have experience of working with customers with complex mental health needs, including self-harm, personality disorder and schizophrenia.

  • Able to work flexibly within a 24hr Rota system, which will include evenings, weekend working and sleep in shifts.
  • Experience of working with customers with complex mental health needs.
  • Self-motivated and able to work on own initiative.
  • Experience of partnership-working.
  • Deliver customer-focused support in line with customer hours and service rotas.
  • Ability to create and maintain personalised support plans.
  • Assess risks, write assessments and draw up risk management plans.
  • To regularly hold key worker meetings to review and update customer outcomes.
  • Ability to use Microsoft IT programmes.
  • NVQ Level 2 in Health and Social Care.
  • Experience in supported housing settings.
