About the Role

Trident Reach is seeking a dedicated and compassionate Specialist Mental Health Support Worker to join our team at our supported living rehabilitation service in Shropshire. This role will focus on providing rehabilitation-centered support to individuals with complex and enduring mental health needs within a 24-hour service environment.

As a Specialist Mental Health Support Worker, you will play a vital role in empowering individuals to build successful relationships within their community and develop skills to better manage their health and well-being. Your commitment to safeguarding will ensure the safety and security of our residents while promoting their independence and growth.

Key Responsibilities
  • Deliver exceptional support to each individual, adhering to best practices and promoting a high standard of care.
  • Empower individuals by encouraging their independence and choice in all aspects of their lives.
  • Provide personalized assistance with personal hygiene and appearance as needed.
  • Collaborate with residents to develop meaningful social, leisure, work, education, or other activities that align with their interests and goals. This may include supporting them in utilizing technology to access information and services.
  • Assist residents in meal preparation, including menu planning, budgeting, and cooking.
  • Encourage active community engagement by supporting residents in participating in local events and activities.
  • Collaborate with care coordinators, social workers, and other care providers to facilitate smooth transitions for residents moving on from our services.
  • Build strong relationships with healthcare professionals and agencies to ensure comprehensive support for residents.
  • Take on the key-working role as assigned by your manager, conducting regular key-work sessions, reviewing support plans, and participating in review meetings alongside the resident.
  • Conduct initial and ongoing assessments in collaboration with the resident, manager, and involved agencies.
  • Promote and uphold Trident Reach's policy on Equality and Diversity.
  • Maintain strict confidentiality regarding all residents and staff information.
  • Adhere to Trident Reach's Policies & Procedures and Code of Conduct.
About You

We are seeking a highly motivated individual with a genuine passion for supporting individuals with complex mental health needs.

  • Ability to work flexibly within a 24-hour rota system, including evenings, weekends, and sleep-in shifts.
  • Experience working with individuals experiencing self-harm, personality disorders, and schizophrenia.
  • Self-motivated and able to work independently while also collaborating effectively as part of a team.
  • Proven experience in partnership-working with various stakeholders.
  • Commitment to delivering customer-focused support that aligns with individual needs and service rotas.
  • Ability to create and maintain personalized support plans tailored to each resident's unique requirements.
  • Skills in assessing risks, writing assessments, and developing risk management plans.
  • Experience in conducting regular key worker meetings to review and update resident outcomes.
  • Proficiency in using Microsoft IT programs.
  • NVQ Level 3 in Health and Social Care or equivalent qualification.
