About the Role

We are seeking a skilled Community Mental Health Support Worker to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care and support to individuals with mental health needs.

Key Responsibilities
  • Establish and maintain positive relationships with service users, their families, and colleagues.
  • Deliver evidence-based therapeutic interventions to support service users in achieving their recovery goals, including improving daily living skills, social inclusion, and meaningful occupation.
  • Conduct physical health assessments, including vital signs and phlebotomy, to ensure comprehensive care.
  • Contribute to the planning and delivery of therapeutic activities, promoting independence and recovery.
  • Support service users in accessing community resources and services, promoting their autonomy and well-being.
  • Communicate effectively with the multidisciplinary team, ensuring seamless care coordination.
  • Participate in risk reviews and updates, ensuring the safety and well-being of service users.
  • Collaborate with other Trust services to provide seamless care pathways, admissions, transfers, and discharges.

Person Specification

We are looking for a candidate with:

  • NVQ 3 in (Mental) Health and Social Care, with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or Functional Skills).
  • Experience working in mental health, with a care/support worker role.
  • Desirable qualifications include working in a community setting, phlebotomy, and NVQ in healthcare.
