About the Role

We are seeking a highly motivated and caring individual to join our Community Team as a Community Healthcare Support Worker. As a key member of our team, you will play a vital role in providing Planned and Admission Avoidance care to patients in the community.

Key Responsibilities
  • Assist and support other members of the team in giving care/undertaking phlebotomy to patients within a designated area, under the supervision of a qualified nurse.
  • Develop additional skills within the HCSW job description and support in career progression through accessing formal training and clinical competence.
  • Work collaboratively with the team to deliver excellent standards of care and improve patient outcomes.
Person Specification
  • GCSE Level C+ in English and Maths or Level 2 functional skills in English and Maths or willingness to work towards this.
  • Level 2 in health and social care (or equivalent) or willingness to work towards this.
  • Care Certificate or willingness to undertake.
  • Awareness of accountability issues e.g., confidentiality.
  • Awareness of the role of the Healthcare Support Worker.
  • Knowledge of the role of the health care assistant role in context, including phlebotomy.
  • Knowledge of situations and circumstances that can impact on patient safety and demonstrate understanding of how the healthcare support worker contributes to maintaining patient safety.
  • Good interpersonal skills (written and oral).
  • Ability to communicate effectively with patients and staff both written and orally and demonstrate a level of English language and grammar commensurate with writing in service user case notes.
  • Ability to remain calm when faced with a potentially difficult situation.
  • Able to meet the travel requirements of the post eg access to vehicle and valid UK driving licence.
