Job Summary

An exciting opportunity has arisen for a Healthcare Support Worker to join the Shropshire & Telford multidisciplinary teams in the provision of Immediate and Planned Care to patients in the community.

Main Duties
  • Promote a 'can do' attitude and determination to develop and sustain a dynamic and responsive clinical and administrative support service within a Community Hospital / Community Team setting which delivers best practice.
  • Promote and enhance the health and wellbeing of patients.
  • Promote and improve the patient experience / journey.
  • Empower patients and promote patient shared/self-care where appropriate.
  • Observe the general behaviour, ability and response of patients/clients and to record the information and report to qualified staff as and when required.
