About the Role

We are seeking a skilled and compassionate Healthcare Support Worker to join our multidisciplinary teams in Shropshire and Telford. As a key member of our community healthcare team, you will play a vital role in providing high-quality care and support to patients in their own homes and community settings.

Main Responsibilities

  • Provide clinical and administrative support to healthcare professionals, promoting a patient-centered approach to care.
  • Assist with the delivery of nursing and therapy services, enabling patients to achieve their full potential and maintain independence.
  • Develop and sustain a dynamic and responsive clinical and administrative support service, promoting best practice and excellence in care.
  • Empower patients and promote patient shared/self-care, enhancing their overall health and wellbeing.
  • Observe and record patient behavior, reporting to qualified staff as necessary, and dealing appropriately with interruptions and unpredictable events.

Requirements

  • GCSE Level C+ in English and Maths or Level 2 functional skills in English and Maths or willingness to work towards this.
  • Level 2 in health and social care (or equivalent) or willingness to work towards this.
  • Care Certificate or willingness to work towards this.
  • Awareness of accountability issues, confidentiality, and the role of the Healthcare Support Worker.
  • Ability to meet the travel requirements of the role.
