Job Summary

The Midlands Partnership NHS Foundation Trust is seeking a skilled and compassionate Mental Health Support Worker to join our team. As a Community Care Specialist, you will play a vital role in providing support and therapeutic time to individuals with mental health needs.

Main Responsibilities
  • Provide individualized support and spend therapeutic time with allocated service users, promoting their recovery and assisting them to maintain their place in the community.
  • Collaborate with internal and external agencies to develop and implement individualized care plans, ensuring seamless transitions between services.
  • Assist the Shift Co-ordinator/Lead Professional in assessing, planning, implementing, and evaluating individualized care plans.
