About This Role

We are seeking a dedicated Community Discharge Assessor to join our Integrated Hospital Discharge Team, providing vital support seven days a week, including weekends and bank holidays. You will play a crucial role in ensuring safe and effective patient flow within our Trust.

Your Responsibilities

  • Collaborate closely with multidisciplinary health and social care teams across acute and community settings to coordinate patient pathways and facilitate safe discharges.
  • Utilize a telephone triage system to assess individual patient needs for discharge, completing Transfer of Care documents accurately and efficiently.
  • Work flexibly within the team to meet service demands, covering a seven-day period including weekends and bank holidays.
  • Support ward professionals in determining appropriate discharge pathways, adhering to established policies and professional guidelines.
