Community Mental Health Support Worker

We are seeking a highly skilled and compassionate Community Mental Health Support Worker to join our team at Midlands Partnership NHS Foundation Trust.
Key Responsibilities- Establish and maintain relationships with service users, relatives, and colleagues to provide high-quality support and care.
- Deliver therapeutic interventions, in accordance with care plans, to assist service users in achieving their recovery goals, including improving activities of daily living, social inclusion, and meaningful occupation.
- Conduct physical health assessments, including weight, height, blood pressure, ECG, and phlebotomy, to ensure comprehensive care.
- Contribute to planning and delivering therapeutic activities with identified groups of service users as part of the care program approach.
- Support service users in accessing a range of activities and services to maintain their independence and promote recovery.
- Ensure that own actions support the equality, diversity, rights, and responsibilities of individuals and promote independence.
- Regularly report and communicate service users' health status and care aims in care records and verbal reports to care coordinators and other members of the multi-disciplinary team.
- Awareness of community resources appropriate for service users to access.
- Involvement in the review of risk, communicating any changes to key workers.
- Work with other Trust services to promote seamless service user-focused care, admission, transfer, and discharge on identified care pathways.
