Job Overview

We are seeking to recruit a Community Care Evaluator to join our Integrated Hospital Discharge team, which operates a comprehensive service throughout the week, including weekends and public holidays. The successful candidate will collaborate with health and social care professionals to determine the most suitable care pathway to facilitate patient discharge. A valid registration as a Clinical or Social Work professional (RN, RMN, AHP, HCPC) is essential, along with proven experience in patient interactions in both hospital and community environments.

Main Responsibilities

The individual in this role will work in partnership with multidisciplinary teams within the Integrated Discharge Team, spanning both acute and community healthcare settings. Key responsibilities include:

  • Completing Transfer of Care documentation through a telephone triage system, evaluating individual patient needs for discharge.
  • Assisting ward staff in establishing discharge pathways in line with established policies and professional standards.
  • Adapting work schedules to meet service demands, ensuring coverage throughout the week, including weekends and public holidays.
  • Ensuring safe and efficient patient flow within the Trust at all times.

The Integrated Discharge Team comprises Health and Social Care professionals, Social Workers, Community Teams, and Local Authorities, all dedicated to ensuring timely and appropriate patient discharges. Person Specification

Qualifications

Essential

  • Degree-level qualification or substantial equivalent experience.
  • Registered Clinical or Social Work professional qualification (RN, RMN, AHP, HCPC).

Desirable

  • Evidence of ongoing professional development.
  • Willingness to pursue further personal development opportunities.

Experience, Knowledge, and Skills

Essential

  • Demonstrated experience in patient interactions in both acute and community settings.
  • Experience within the NHS, local authority, or health and social care sectors.
  • Ability to exercise sensitivity and empathy when engaging with patients and their families, particularly in emotionally charged situations.
  • Strong organizational skills.
  • Experience in developing systems and processes to enhance patient flow.
  • Understanding of current mental health legislation and safeguarding protocols.
  • Experience collaborating with multidisciplinary health and social care teams.
  • Excellent communication abilities.
  • Capacity to demonstrate initiative and prioritize tasks effectively under pressure.

Desirable

  • Aware of the importance of confidentiality in accordance with the Data Protection Act and relevant guidelines.
  • Experience with discharge coordination processes.
  • Ability to conduct comprehensive health, social, or therapy assessments.
  • Understanding of the needs of frail and complex patients.
  • Experience in individual goal planning to support patients in achieving their recovery objectives post-hospitalization.
  • Knowledge of Hospital Discharge Requirements, Discharge to Assess, Care Act, and Mental Health Act.
  • Experience in community settings, including domiciliary care or care homes.
