Position Overview

Clinical Nurse Evaluator - Flexible Office Hours with Travel for In-Person Evaluations

Compensation: £40,000 - £45,000

Gray Healthcare is a dedicated provider in Health and Social Care, specializing in assisting individuals with intricate needs, disabilities, and chronic conditions as they transition back into community living from inpatient care. Our innovative approach to care empowers individuals to regain control over their lives by facilitating their return to their homes and communities. We design personalized support plans tailored to each individual, backed by our comprehensive multi-disciplinary team, ensuring adaptability to meet both current and future requirements.

Role Objectives

Reporting to the Head of Nursing, you will spearhead clinical evaluations to guarantee the delivery of customized support plans. Collaborating closely with the Assessment and Contracts Manager, you will ensure that these plans are feasible and align with our support model across a designated geographical region.

Key Responsibilities:

  • Conducting both desk-based and in-person evaluations of individuals referred for support.
  • Implementing an APIE methodology - Assessing, Planning, Implementing, and Evaluating.
  • Reviewing documentation and evidence of outcome measures to inform clinical practices, business performance, and quality standards. Maintaining continuous professional development and being prepared to present your CPD portfolio upon request.
  • Identifying and mitigating complex risks within community settings.
  • Timely and professional review of pre-admission assessments.
  • Producing clinically sound evaluations with thorough risk assessments and identifying risk mitigation strategies.
  • Recognizing strategies to empower individuals through positive support.
  • Participating in internal and external MDT meetings to enhance informed evaluations and risk management.
  • Fostering professional relationships.
  • Managing time effectively.
  • Maintaining accurate, safe, and contemporaneous records, and collaborating effectively with stakeholders and external professionals.

To qualify for this position, candidates should meet the following criteria:

  • Registered Mental Health Nurse, Occupational Therapist, or equivalent qualification.
  • Experience in assessing and supporting individuals with diverse complex needs related to mental health, learning disabilities, acquired brain injuries, personality disorders, autism, and forensic histories.
  • Knowledge of continuing healthcare or the transition of individuals from hospital/residential settings to community living, with a collaborative and solutions-oriented approach to addressing related challenges.
  • Practical understanding of the Mental Health Act, Community Treatment Orders, and the Mental Capacity Act, including Deprivation of Liberty Safeguards and Best Interest Decision-making.
  • An expert in risk assessment and management, possessing innovative thinking, strong communication skills, proficient report writing abilities, and computer literacy, with the capacity to perform under pressure.
  • Willingness to travel for in-person evaluations within an agreed geographical area.
  • Valid UK Driving Licence and access to a vehicle.
