Job Overview

Job Duties

- Monitor and assess patient care and treatment progress in accordance with Trust and local policies.

- Manage patient pathways effectively and liaise with clinicians as necessary.

- Ensure precise data entry and maintain accurate records.

- Identify subsequent steps in the patient care pathway based on clinical requirements.

- Process patient referrals in alignment with established timelines and requirements.

- Provide administrative support to relevant teams, ensuring all necessary documentation and clinical information is accessible.

- Respond to inquiries regarding patient care and treatment pathways in a timely manner.

- Develop familiarity with the milestones of the assigned patient pathways.

- Attend relevant meetings and follow up on actions related to patient care.

- Participate in national audits and gather data as needed to enhance treatment outcomes.

- Propose improvements to the patient care pathway and tracking processes.

- Monitor patient progress and collaborate with clinicians and managers to expedite treatment.

- Address daily operational issues, ensuring resolution and escalating significant concerns to management.

- Facilitate the efficient transfer of patients and information between trusts and organizations involved in the patient pathway.

- Collaborate within a multi-disciplinary team to develop services in line with departmental and Trust objectives.

- Ensure health records are tracked and stored securely according to policy.

- Cultivate working relationships with a diverse range of clinical and managerial personnel.

- Responsible for maintaining and updating patient information in relevant databases to ensure efficient pathway management.

- Engage in departmental development initiatives and participate in meetings.

- Undertake any other duties necessary for the smooth operation of the department.

- Complete required training to fulfill job responsibilities.

Person Specification

Qualifications/ Education

Essential

GCSEs or equivalent, including Mathematics and English.

Desirable

Qualification or training in customer service.

Experience

Essential

Demonstrated administrative experience.

Desirable

Experience in an administrative role within the NHS. Experience in supervising staff.

Skills/Knowledge/Ability

Essential

Proficient IT skills, particularly in Microsoft Office applications, including Word, Outlook, and Excel. Strong organizational and time management skills to meet deadlines. Ability to manage challenging interactions with service users. Excellent interpersonal skills, with the ability to communicate effectively with a diverse range of multi-disciplinary staff.
