Role Overview:
The Patient Pathway Administrator will oversee the management of patient pathways and scheduling.

By collaborating with a diverse array of clinical and administrative personnel, and delivering exceptional administrative assistance, the individual in this role will ensure that patients receive timely treatment and that national benchmarks are achieved.


The incumbent is tasked with providing a high level of professional and patient-centered service across various specialties within the Trust, supporting daily operational functions.

The responsibilities outlined below are indicative of the Patient Pathway Administrator position and its role within the service; they are not exhaustive, and the post holder may be required to perform additional or alternative administrative and managerial tasks as necessary to facilitate the efficient operation of the department.

  • Monitor and assess patient care and treatment progress using the Patient Tracking List in accordance with Trust and local policies.
  • Identify subsequent steps in the patient care pathway based on clinical requirements.
  • Process patient referrals and manage them according to established timelines and requirements.
  • Coordinate and provide administrative support to relevant teams to ensure all necessary documentation and clinical information is accessible.
  • Respond to inquiries regarding patients' care and treatment pathways from external Trusts and General Practitioners promptly.
  • Develop a comprehensive understanding of the key milestones within the assigned patient pathways.
  • Attend pertinent meetings and fulfill action items related to patient care within the designated pathways.
  • Participate in national audits and gather data as needed to assist the Trust in enhancing treatment for patient groups.
  • Propose and identify enhancements to the patient care pathways and tracking processes.
  • Monitor patients along their pathways and proactively collaborate with clinicians and managers to expedite treatment.
  • Report any issues or breaches of Waiting Times standards to the appropriate management in line with established procedures.
  • Ensure the efficient transfer of patients and information between trusts and other organizations involved in the patient pathways, adhering to agreed communication protocols.
  • Collaborate within a multidisciplinary team to develop services in alignment with departmental plans and Trust objectives.
  • Ensure all health records are accurately tracked and securely stored in compliance with Health Records Policy.
Communication:
  • Cultivate professional relationships with a diverse range of clinical and managerial staff.

Data Management:
  • Responsible for collecting and updating patient information on relevant databases in accordance with local policy to ensure efficient pathway progression.
  • Collaborate with the team to analyze target breaches and communicate issues to management.
Policy Development:
  • Assist in the review and implementation of Standard Operating Procedures.
  • Support the development and implementation of internal targets.
Additional Responsibilities:
  • Perform any administrative tasks necessary to support the delivery of high-quality patient care.
  • Actively engage in departmental development and participate in meetings.
  • Undertake any other duties as required to ensure the smooth operation of the department.
  • Complete any required training to fulfill the responsibilities associated with the role.
