Main Responsibilities
The primary role involves serving as the main contact for patients regarding their care pathways, both for inpatients and outpatients. The post holder will collaborate within a multidisciplinary team under the guidance of the Clinical Lead, providing expertise in the proactive management of patient pathways from referral to discharge. This role is crucial in ensuring a seamless experience for patients and clinicians alike.
The successful candidate will maintain high levels of patient and clinician satisfaction by being an accessible, knowledgeable, and customer-focused point of contact. To ensure consistency across the Trust, Standard Operating Procedures (SOP) will outline the functions of this role and the associated timelines. As a Trainee, the post holder will perform their duties under supervision.
