**Job Overview**

We are seeking a highly organized and detail-focused individual to join our team as a Patient Pathway Coordinator. In this role, you will be responsible for tracking patient pathways, processing referrals, and managing data on internal databases. You will work closely with clinical and managerial staff to ensure patients receive timely care and national targets are met.

**Key Responsibilities**

* Track patient pathways and manage referrals
* Collect and update patient information on internal databases
* Provide day-to-day supervision and oversight of departmental administrators
* Ensure adherence to Trust policies and protocols
* Identify training and development needs of staff

**Requirements**

To be successful in this role, you will need to have significant experience in administration, particularly in a fast-paced customer care environment. You will also need to have strong IT skills, including proficiency in Microsoft Office packages.
