Your Role as a Care Coordinator

As a Care Coordinator, you will play a vital role in ensuring our clients receive the highest quality care. You will be responsible for managing schedules, coordinating with care professionals, and building strong relationships with both clients and their families.

Key Responsibilities
  • Schedule Management: Develop and maintain detailed schedules for our care professionals, ensuring optimal coverage and client needs are met.
  • Client Communication: Build rapport with clients and their families, providing clear communication regarding schedules, updates, and any necessary information.
  • Care Professional Liaison: Coordinate with our dedicated team of care professionals, addressing scheduling requests, providing support, and ensuring smooth operations.
  • Problem Solving & Adaptability: Respond effectively to changes in schedules or client needs, demonstrating flexibility and resourcefulness in finding solutions.
  • Relationship Building: Foster positive relationships with clients, care professionals, and internal teams, creating a collaborative and supportive environment.
Qualifications & Skills

We are seeking an organized and compassionate individual with excellent communication and interpersonal skills.

  • Proven experience in scheduling or coordination within a healthcare setting is preferred.
  • Strong problem-solving abilities and the ability to work effectively under pressure.
  • Excellent organizational and time management skills.
  • A genuine passion for providing exceptional care and support to seniors and individuals with disabilities.
