Position Overview

This is a remarkable opportunity for a dedicated and proactive Band 6 Community Nurse to become part of the Adult Community Nursing Team, providing flexible care throughout the week. The Specialist Practitioner Qualification in District Nursing is preferred, along with significant experience at the Band 5 level within community nursing.

The Swinton Community Nursing Team offers care to individuals in their residences. We operate as part of a dynamic, integrated health and social care multidisciplinary team, delivering high-quality support, guidance, and monitoring to patients, their families, and staff in community settings. Under the supervision of the District Nurse Lead, you will deliver care to patients with both complex and straightforward needs, aiming to foster independence and self-management.

Exceptional communication skills are essential for this role.

You will receive support from a skilled team in an environment that actively promotes training and development opportunities to meet the clinical demands of the position. We provide a thorough induction program, and you will undergo annual appraisals to enhance your knowledge and skills as needed.

Main Responsibilities

Your responsibilities will include providing nursing care to patients in the community and organizing the workload for junior staff while supporting the team leader.

You will engage in delivering nursing care and assistance to clients in the community, demonstrating the confidence and competence to work independently.

Under the guidance of the Integrated Team Leader, you will be accountable for assessing and managing the health needs of a designated population, including end-of-life care and patients with long-term conditions or complex nursing requirements.

Strong communication and interpersonal skills are vital, as you will frequently be required to lead team safety discussions, participate in multidisciplinary meetings, and contribute to discharge planning to ensure seamless patient care.

The role entails assessing patient care needs and developing, implementing, and evaluating care programs for specific patient groups while supporting the care of all patients. As a Sister/Charge Nurse, you will provide daily clinical and managerial leadership to nursing and support staff, ensuring the delivery of evidence-based practice.

Essential Qualifications

Applicants must possess a current NMC PIN to practice as a Registered Nurse or Midwife in the UK, or be enrolled in a course that will lead to obtaining a full NMC PIN within the next three months. Please note that an NMC PRN PIN does not meet the essential criteria for this role.

The NCA is currently unable to accept applications from International Nurses and Midwives who have not completed all competencies required for a full NMC PIN to practice in the UK, as our OSCE program is on hold indefinitely.
