About you

  • Experience in understanding the needs of service users in palliative care/long-term conditions.
  • Clinical leadership skills.
  • Able to quickly respond to problems, including identifying and implementing solutions.
  • An ability to use own initiative in unsupervised settings.
  • An ability to provide teaching and mentorship.
  • Effective verbal and written communication skills, including an ability to communicate difficult messages.
About the role

We have an exciting new vacancy for an Admissions and Discharge Coordinator to support the hospice.

This role is to lead on improving and sustaining our admissions and flow in the hospice across community and the ward.

It is to work with stakeholders in the hospital, community and the hospice from clinical staff through to executives in the ICB and beyond.

Elements of the role are to review the referrals and triage as well, working with the Doctors and Nurses in Charge to manage this independently.

A further part of the role is to work within discharge planning, we are a short stay service, so it is to continually review our key quality indicators and develop good discharge processes with the teams.


  • The post holder will need to provide clinical leadership, mentor and supervision of junior staff (no line management).
  • Responsible for fostering and managing good relationships with internal and external stakeholders, including meetings and coordination off site.
  • Acting as a key co-ordinator for admissions and discharges, supporting workflows to improve these areas.
  • Responsible for actively leading, directing, supporting, and facilitating the identification of service users and increasing our reach, by project leading the admissions and operations work group.
  • Ensuring all people have an active preferred place of care and death and their needs are met.
