About the Role

We are seeking a highly skilled and experienced Patient Care Coordinator to join our team at Gloucestershire Hospitals NHS Foundation Trust. As a key member of our discharge planning team, you will play a vital role in ensuring the smooth transition of patients from our acute hospital to their onward care providers.

Key Responsibilities
  • To independently case manage the discharge planning process for a cohort of patients within a defined clinical area, from admission to discharge.
  • To work closely with multidisciplinary teams to ensure that patients receive the necessary support and resources to meet their expected date of discharge.
  • To act as a resource providing advice and information to clinical teams to enable patients with onward care requirements to meet their expected date of discharge safely and effectively.
  • To assist with the assessment of patients with the support of a qualified practitioner when needed.
  • To work as part of the ward multi-disciplinary team, leading pre-emptive discussions on discharge planning.
Person Specification

Essential

  • Experience of working in an acute clinical environment.
  • Previous experience in a relevant speciality.

Desirable

  • Demonstrate an understanding of the roles of the different members of the MDT.
  • Able to contribute to the implementation of policies/protocols/guidelines relevant to area of practice.
