About the Role

We are seeking a highly skilled and experienced Senior Patient Flow Coordinator to join our team at Gloucestershire Hospitals NHS Foundation Trust. In this role, you will play a critical part in ensuring the smooth transition of patients from acute care to community-based services.

Key Responsibilities
  • Support members of the multidisciplinary team to work effectively in discharge planning.
  • Ensure timely access to information, advice, and services to support discharge, including equipment ordering and onward referrals.
  • Assist with patient assessments, working closely with qualified practitioners as needed.
  • Lead pre-emptive discussions on discharge planning within the ward multidisciplinary team.
  • Communicate effectively with people from all professions and levels within our organisation and beyond.
Person Specification
  • Experience of working in an acute clinical environment.
  • Previous experience in a relevant speciality.
  • Ability to act as a role model and lead by example, reinforcing the Trust's values and behaviours.
  • Effective communication and escalation skills to reduce risk and promote patient safety.
