About the Role

We are seeking a highly skilled and experienced Senior Patient Flow Coordinator to join our team at Gloucestershire Hospitals NHS Foundation Trust. As a key member of our Onward Care Team, you will play a vital role in ensuring the smooth transition of patients from hospital to community care.

Key Responsibilities

  • To independently case manage the discharge planning process for a cohort of patients within a defined clinical area, from admission to discharge.
  • To coordinate the provision of Onward Care Team cover to ward areas within their team, in the absence of a Senior OCT clinician.
  • To act as a resource, providing advice and information to clinical teams to enable patients with onward care requirements to meet their expected date of discharge safely and effectively.

Person Specification

  • Essential
    • Experience of working in an acute clinical environment.
    • Previous experience in a relevant specialty.
  • Desirable
    • Demonstrate an understanding of the roles of the different members of the Multi-Disciplinary Team.
    • Able to contribute to the implementation of policies, protocols, and guidelines relevant to the area of practice.
