About the Role

We are seeking a highly skilled and experienced Patient Care Coordinator to join our team at Gloucestershire Hospitals NHS Foundation Trust. As a key member of our discharge planning team, you will play a vital role in ensuring the smooth transition of patients from our acute hospital to their onward care providers.

Key Responsibilities
  • Support members of the multidisciplinary team to work effectively in discharge planning.
  • Ensure information, advice, and services are available in a timely manner to support discharge, including ordering of equipment and onward referral when needed.
  • Assist with the assessment of patients with the support of a qualified practitioner when needed.
  • Work as part of the ward multidisciplinary team, leading pre-emptive discussions on discharge planning.
  • Communicate effectively with people from all professions and levels within our organisation and beyond.
Requirements
  • Experience of working in an acute clinical environment.
  • Previous experience in a relevant specialty.
  • Ability to act as a role model and lead by example to ensure the Trust's values and behaviours are reinforced throughout their area of practice.
  • Ability to effectively and appropriately escalate concerns to reduce risk and promote patient safety.
