Job Summary

The Onward Care Team is expanding, and we are seeking a highly skilled and motivated individual to join our team as a Senior Patient Flow Coordinator. In this role, you will have the opportunity to work with a diverse range of healthcare professionals and patients, and contribute to the development of effective discharge planning processes.

Main Responsibilities
  • Support members of the OCT and ward staff to work effectively in discharge planning.
  • Ensure information, advice, and services are available in a timely way to support discharge, including ordering of equipment and onward referral when needed.
  • Assist with the assessment of patients with the support of a qualified practitioner when needed.
  • Work as part of the ward multi-disciplinary team, leading pre-emptive discussions on discharge planning.
About Us

We are a team of therapists, nurses, and non-registered clinicians who work across both sites within Gloucestershire Hospitals NHS Foundation Trust to support the coordination of onward care planning for those patients who require some support when leaving hospital.

Person Specification
  • Experience of working in an acute clinical environment.
  • Previous experience in a relevant speciality.
  • Act as a role model and lead by example to ensure the Trust's values and behaviours are reinforced throughout their area of practice.
  • Able to effectively and appropriately escalate concerns to reduce risk and promote patient safety.
