Job summary

The post holder will provide day-to-day clinical nursing/therapy expertise and co-ordination to a multi-disciplinary integrated discharge hub based at the Newham General University Hospital working inpartnership to facilitate timely discharge of patients from hospitals. The team consists of nurses,therapists, social workers and admins.

The post holder will:

o Undertake 'in reach' triage assessments for patients on the acute Wards at NewhamUniversity Hospital to ensure rapid discharge. Undertake generic IDH clinical assessment andcoordination tasks and functions.

o To coordinate and chairing of the IDH Clinical Discussion meetings face to face or virtuallyheld at different time weekdays and weekend with system partners and discharge hubs.

o Liaison with the Discharge Coordinator Team in NUHT and other system partners, dischargehubs to identify and facilitate the discharge of patients from the acute hospital setting tocommunity services

o Effective liaison and maintain relationships with the Hospital Social Work Team and othersystem partners to ensure that appropriate equipment, Packages of Care and support systemare put in place to meet the health and social care needs of patients discharged via the IDH.

o The post holder will be an active participant in shift work, covering either 8am-4pm, 9am-5pm,10am-6pm or 12-8pm, with rota'd weekends and Bank Holidays as the service operates 365days per year.

Main duties of the job

To maintain clinical direction and leadership; coordinating a team of clinical staff for the continuous and consistent provision of high-quality patient care in acute and in the community.

To proactively maintain safe guarding knowledge and ensure risk assessment, co-ordinate, review, case manage and facilitate discharges into community services as part of the integrated discharge hub.

To be aware of professional boundaries and the need to access expert and specialist opinions when required in relation to discharge planning.

To demonstrate within the team clinical leadership and expert specialist knowledge of a range of therapeutic and clinical interventions in order to plan discharge, undertaking assessments equipment and evidence based recommendation. Escalate clinical risks as appropriate .

Act as a role model in the practice area, encouraging staff in teaching, supervision, facilitating and assessing, create and sustain a supportive/teaching/learning environment for the practice setting.

Be responsible for the maintenance and accurate recording of patient information in the documentation systems and to ensure the effective dissemination of patient information throughout the multi-disciplinary team.

The post holder will be required to work with partners across the health and social care to ensure a joined up and integrated approach is adopted towards the discharge process and provide holistic care.

The post holder will support and oversee the team management, co-ordination and provision of service. The post holder will involve in supervision and development of junior staffs.

The post holder will be an active participant in shift work, covering either 8am-4pm, 10am-6pm, 9am-5pm or 12-8pm with rotad weekends and Bank holidays as the service operates 365 days per year.

Education/ Qualification/ Training

Essential

oDiploma or Degree in Physiotherapy or Occupational Therapy. oEvidence of post-qualifying and continuing professional development oRegistered with the HCPC

Experience

Essential

oWell-developed and suitable experience of working as a qualified in adult community mental health services oClinical/discharge experience oExperience in screening and triaging referrals and discharge planning oExperience of contributing to the professional development and education of more junior staff, students and assistants oExperience of working in rehabilitation, social services and community based work oExperience working in MDT

Desirable

oWell-developed experience in working with advanced physical assessment, preferably in an acute and community setting oExperience of Clinical Leadership, oExperience of clinical supervision, mentoring/coaching and staff management including appraisal oExperience of performance management oInvolvement in audit, research and understanding of quality issues
