Opportunity Summary

We are seeking a talented and empathetic Community Mental Health Support Worker to join our team as a Duty Practitioner. In this role, you will have the opportunity to work closely with clients, families, and multidisciplinary teams to provide high-quality support and care.

Your primary responsibilities will include:

  1. Providing emotional support and reassurance to clients in need;
  2. Assessing clients' mental health needs and developing personalized treatment plans;
  3. Coordinating care services and connecting clients with community resources;
  4. Participating in clinical meetings and contributing to the development of innovative solutions to complex problems;
  5. Maintaining accurate records and communicating effectively with multidisciplinary teams.

Salary: £28,000 - £38,000 per annum (dependent on experience)
