Job Summary

We are seeking a highly skilled and experienced Care Manager Lead to join our team at Care UK.

About the Role

As a Care Manager Lead, you will be responsible for leading and supervising your shift, case managing residents, and liaising with families and healthcare professionals to ensure the best care plans are created and delivered.

Key Responsibilities
  • Leading and supervising your shift, ensuring that residents receive high-quality care and support.
  • Case managing residents, including assessing their needs and developing personalized care plans.
  • Liaising with families and healthcare professionals to ensure that residents receive comprehensive care and support.
  • Administering and recording medication, as required.
  • Carrying out risk assessments and implementing measures to minimize risks.
  • Working closely with care assistants to support residents with daily needs and promote independence.
  • Collaborating with the leadership team to develop services and improve resident outcomes.
Requirements
  • NVQ Level 3 in health and social care or equivalent experience.
  • Proven ability to lead and supervise others, with excellent communication and interpersonal skills.
  • Strong understanding of care planning processes and medication administration.
  • Able to work effectively in a fast-paced environment and prioritize tasks accordingly.
  • Commitment to delivering exceptional care and support to residents, with a strong focus on resident-centered care.
