Healthcare Coordinator

At Intermountain Healthcare, we are seeking a skilled Healthcare Coordinator to join our team. In this role, you will work collaboratively with physicians and other team members to provide exceptional patient care and support.
This position requires strong communication, service coordination, and problem-solving skills to ensure optimal patient outcomes. You will facilitate communication, coordinate services, and address barriers to deliver high-quality care.
Key Responsibilities:
- Patient Population Identification using specific screening criteria
- Care Plan Development and Maintenance for members receiving care management services
- Communication Facilitation among healthcare team members and other relevant parties
Requirements:
- Masters Degree in Social Work from an accredited institution
- Current State Licensure as a Certified Social Worker (CSW) or Licensed Clinical Social Worker (LCSW)
- Basic computer skills and knowledge of word processing and spreadsheet software
Salary: $34.06-$52.55 per hour (dependent on experience)
Benefits: Benefits Eligible, Part-time, Remote position with option to work at Valley Center Tower
