Healthcare Coordinator Specialist

We are seeking a skilled Healthcare Coordinator Specialist to join our team at Intermountain Healthcare. As a member of our team, you will play a critical role in coordinating patient care and ensuring seamless transitions between healthcare services.
This is a fully remote position, allowing for flexibility and work-life balance. You will work with our Trinsic department, an accountable care organization that focuses on Value-Based Care programs. Your experience in healthcare, particularly in areas such as medical assisting, care management, social determinants of health, primary care practices, value-based care, quality metrics, and public speaking, will be highly valued.
In this role, you will assess and document patients' socio-economic reviews and screenings to support coordination of care. You will plan interventions to help patients navigate the complex healthcare system and remove barriers to care. You will also provide clinical support and coordination related to Value-Based Care programs and work in an advisory capacity to support standards of care.
Key Responsibilities:
* Participate in huddles with primary care practices to assist with scorecard initiatives and program metrics.
* Consult with care teams to provide care coordination for preventative care and utilization mitigation.
* Identify appropriate resources, including transportation, housing, healthcare, and social/spiritual services as needed.
Physical Requirements:
This role requires ongoing need for employees to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors require verbal communication, hearing, and understanding spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers is required to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
The hourly range for this position is $18.38 - $26.65.
