Job Overview
As a Care Coordination Specialist with Intermountain Health, you will play a vital role in supporting the health and well-being of our patients. This is a fully remote position that requires strong communication skills and the ability to work independently.

**Key Responsibilities:**
* Assess and document patients' socio-economic reviews and screenings to support care coordination
* Plan interventions to help patients navigate the complex healthcare system and reduce barriers to care
* Provide clinical support and coordination related to value-based care programs

Requirements and Qualifications:
* 2+ years of experience in healthcare as a Medical Assistant, Care Manager, or Social Determinants of Health specialist
* Strong understanding of quality metrics and public speaking skills
* Ability to participate in huddles with primary care practices and provide care coordination for preventative care and utilization mitigation
