Care Delivery Specialist

Care Coordination Specialist - Are you a compassionate and detail-oriented Registered Nurse looking for a new challenge? We invite you to join our team as a Nurse Care Coordinator at Centene Corporation.
In this role, you will work closely with our healthcare team to develop and implement care plans that meet the unique needs of each member. Your responsibilities will include assessing member needs, identifying barriers to care, and collaborating with healthcare providers to ensure effective care coordination.
- Evaluate member needs and identify barriers to care.
- Develop and implement individualized care plans.
- Collaborate with healthcare providers to address unmet needs.
- Provide education and support to members and caregivers on disease management and healthcare options.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
A Bachelor's degree in Nursing and 2-4 years of related experience are required. You must have a valid RN license and compact state licensure.
Pay Range: $54,000 - $97,100 per year
