Healthcare Coordinator Professional

Care Management Specialist - Join us at Centene Corporation and make a difference in the lives of our 28 million members. As a Nurse Care Coordinator, you will be responsible for developing and implementing personalized care plans that address the unique needs of each member.
This role is perfect for individuals who are passionate about delivering high-quality, patient-centered care. Our ideal candidate will have a strong background in nursing and experience working with vulnerable populations.
- Develop, implement, and evaluate individualized care plans.
- Collaborate with healthcare providers to address unmet needs and promote effective care coordination.
- Conduct regular assessments to identify member needs and barriers to care.
- Provide education and support to members and caregivers on disease management and healthcare options.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
A minimum of 2-4 years of experience as a Registered Nurse is required. You must have a valid RN license and compact state licensure.
Pay Range: $54,000 - $97,100 per year
