Dementia CPN Role

We are seeking an experienced Mental Health Nurse to join our Community Mental Health Team for Older People (CMHT OP) as a specialist in adult dementia care.
The successful candidate will have a strong background in community mental health nursing, with experience in older people's care coordination and caseload management.
This role offers excellent opportunities for professional development and growth, with a competitive salary of £39,205 - £47,084 per year.
As part of our CMHT OP service, you will be responsible for assessment, diagnosis, treatment, and support for individuals over 65 with functional mental health illnesses, including deterioration in cognition and/or dementia.
Our service works closely with other organizations, such as social care and the voluntary sector, to provide positive support for people using services and their carers.
The successful candidate will be expected to:
- Act as a key worker/care coordinator for named individuals
- Provide high standards of care and professional supervision
- Educate junior staff/students on competencies of a Community Mental Health Nurse
