Job SummaryAs a key worker, you will be responsible for assessing, diagnosing, treating, and supporting individuals over 65 with complex needs. You will work closely with other services, including social care and the voluntary sector, to provide positive support and forward signposting.
Your role will involve developing close working relationships with statutory and voluntary services, educating junior staff, and participating in integrated team working. A strong emphasis is placed on collaboration and communication to deliver high-quality care and support.
Key Responsibilities- Develop and implement personalized care plans for individuals with severe mental health problems and dementia
- Work collaboratively with multidisciplinary teams to achieve positive outcomes for service users
- Provide education and training to junior staff to enhance their skills and knowledge
- Participate in Duty Rota to manage new referrals and ensure timely interventions
The salary for this role is £39,205 - £47,084 per annum (inclusive). We offer a comprehensive benefits package, including opportunities for professional development and career progression.
