The Surrey and Borders Partnership NHS Foundation Trust is seeking a highly skilled Community Psychiatric Nurse to join their team. As a specialist in dementia care, you will be responsible for providing comprehensive assessments, developing shared understanding of challenging behaviors, and collaborating with care home teams.

Main Responsibilities:
  • Provide therapeutically-informed assessments for individuals with dementia and functional diagnoses residing in care homes.
  • Develop and implement personalized care plans to address challenging behaviors.
  • Collaborate with multidisciplinary teams, including General Practitioners, primary care teams, and social services.

This role offers opportunities for professional growth, training, and supervision. You will be part of a supportive nursing network and have access to rolling internal CPD programs. The salary for this position is £37,338 to £44,962 per annum.
