Job OverviewWe are seeking an experienced Community Psychiatric Nurse to join our team as a Mental Health Specialist. The successful candidate will work closely with colleagues from various professional backgrounds to provide specialist input to the Care Home Pathway within our Community Mental Health Team for Older People (CMHTOP).
This role offers an exciting opportunity to make a positive impact on the lives of individuals living in care homes, particularly those with dementia who exhibit challenging behaviors. As a specialist advisor, you will work collaboratively with care home teams and other relevant agencies to develop shared understandings of complex behaviors and provide comprehensive, therapeutically-informed assessments.
Main Responsibilities- Provide comprehensive assessments for individuals with dementia and functional diagnoses residing in care homes.
- Develop shared understandings of challenging behaviors and communicate effectively with care home teams and other relevant agencies.
- Complete complex risk assessments and refer or signpost to other appropriate services as needed.
- Participate in the wider CMHTOP duty rota and collaborate with multidisciplinary teams.
- Act as a support and specialist advisor to other services, including General Practitioners, primary care teams, and care home services.
- Provide direction, supervision, and appraisals for junior staff and mentor junior colleagues wishing to gain experience in dementia care or working with older people.

Requirements- RMN/BSC Hons or Diploma in mental health nursing
