Advanced Dementia Specialist Nurse

We are seeking a highly skilled Community Mental Health Nurse to provide exceptional care to individuals over 65 with severe mental health problems and dementia.
About the RoleThis is an exciting opportunity for a dedicated professional to work in a highly regarded team, delivering high-quality care and support to vulnerable individuals. As a key worker, you will be responsible for assessing, diagnosing, treating, and supporting individuals with complex needs.
You will work closely with other services, including social care and the voluntary sector, to provide positive support and forward signposting. Your role will involve developing close working relationships with statutory and voluntary services, educating junior staff, and participating in integrated team working.
Main Responsibilities- Act as a key worker/care co-ordinator for named individuals, within the Care Programme Approach process
- Provide high standards of care in a professional manner
- Ensure care delivered is based on best practice and regularly reviewed
- Supervise juniors/staff in delivery of care and treatment
- Support junior staff to manage caseloads effectively
- Participate in Duty Rota to manage new referrals
The salary for this role is £39,205 - £47,084 per annum (inclusive). The post also offers a range of benefits, including opportunities for professional development and career progression.
