Job Title: Administrative Officer - Care Coordination Specialist

Job Summary:

We are seeking an experienced Administrative Officer to join our Brokerage Team as a Care Coordination Specialist. In this role, you will be responsible for providing administrative support to our team, including sourcing and establishing person-centred care arrangements for individuals.

Key Responsibilities:

  • Referral Management: Receive and process referrals from various sources, ensuring timely and accurate processing.
  • Care Sourcing: Identify and negotiate care arrangements with providers, ensuring high-quality and cost-effective solutions for individuals.
  • Liaison and Communication: Develop and maintain relationships with internal stakeholders, including operational teams and external partners, to ensure seamless care coordination.
  • Financial Management: Manage financial provisions and ensure accurate recording of data in our systems.
  • Reporting and Data Analysis: Assist in collecting and recording data to meet reporting requirements, providing insights to inform business decisions.
  • Team Support: Collaborate with team members and management to achieve team objectives and provide administrative support as needed.

Requirements:

  • Education: A minimum of GCSE or equivalent in English and Maths.
  • Skills: Excellent attention to detail, strong IT skills, and ability to work effectively in a team environment.
  • Experience: Previous experience in a similar role or in a related field, with a strong understanding of care coordination and administrative processes.
