Key Responsibilities

The Lead Community Pediatric Nurse is tasked with delivering specialized, proficient nursing care to children and young individuals with diverse healthcare needs, including short-term acute care, long-term health issues, complex conditions, and palliative care, while continuously assessing care requirements.

Our goal is to facilitate timely discharges and avert unnecessary hospital readmissions for children with either acute or chronic health conditions. This is achieved through specialized support and clinical expertise to assist in the assessment, treatment, and care planning, provided by an autonomous practitioner. The role will also involve acting as a resource for pediatric expertise, offering specialized guidance to other professionals and promoting evidence-based practices.

Care will be administered in various settings, including homes, schools, and other community environments, with the expectation that the post holder will develop, implement, and evaluate specialized nursing care programs. Collaboration with partners across NHS organizations, including primary care networks, to enhance an integrated pediatric nursing service will also be part of your responsibilities.

Detailed Responsibilities

The Community Pediatric Nursing Team is dedicated to delivering a high-quality in-reach and outreach nursing service that ensures safe discharges from hospitals and prevents avoidable admissions.

The team maintains strong connections with colleagues in our Specialist Nursing teams, addressing various long-term health conditions such as Endocrine and Diabetes, Respiratory, Allergy, Neuromuscular, Bladder and Bowel, Complex Needs, Palliative Care, and Epilepsy.

Close collaboration with acute pediatric clinical areas and community and neuro-developmental pediatric teams is also a key aspect of our work.

Responsibilities Include:

  • Playing a crucial role in multi-faceted care delivery processes and enhancing partnership working to ensure a seamless patient journey.
  • Regularly developing, implementing, and evaluating individualized care programs.
  • Supporting the management of integrated acute and community care pathways for children with both short and long-term conditions.
  • Educating families and caregivers on specific complex nursing care and procedures, empowering them to confidently care for their child.
  • Performing clinical skills such as blood sampling, central line care, and wound management.
  • Administering medications, including intravenous therapies and cytotoxic drugs, in collaboration with pharmacy colleagues.
  • Maintaining awareness of current medications and therapies for pain management and other symptoms, requiring specialized knowledge in pediatric palliative care and symptom management.
  • Handling sensitive and complex information with empathy and reassurance.
  • Demonstrating exemplary clinical practice, staying updated with new developments in nursing care and treatments, ensuring evidence-based practice through current research.
  • Actively participating in and promoting clinical supervision and reflective practice.
  • Utilizing computer programs effectively and possessing strong IT skills.

Qualifications and Experience

Essential Criteria

  • Valid NMC Registration as a Children's Nurse.
  • Mentorship qualification.

Desirable Criteria

  • Specialist Practitioner Degree in Children's Nursing.

Knowledge and Experience

Essential Criteria

  • Proven clinical competence in the area of Children's Community Nursing, including preparation and administration of IV medications and wound care.
  • Ability to implement changes in nursing practice.
  • Understanding of Clinical Governance and Safeguarding and their relevance to practice.
  • Familiarity with relevant policies and procedures.
  • Experience in caring for children with complex healthcare needs.
