Primary Care Team Leader

Greater Manchester Mental Health is seeking a highly skilled and experienced Band 6 Registered Physical Health Nurse to join our dynamic Primary Care Team as a Team Leader. This is an exciting opportunity for an aspiring leader to take on a key role in delivering high-quality patient care and contributing to the development of our team.
Main Responsibilities- Assist in the overall delivery of care to patients within the Primary Care Department.
- Assess care needs and develop, implement, and evaluate pathways of care.
- Lead and develop a team of healthcare staff and visiting professionals.
- Work towards providing the highest standard of care for the prison population.
- Ensure consistent and effective nursing practice and operational activity.
