Job Summary

The Primary Care Practitioner will be a key member of the Primary Care Team, providing comprehensive primary care services to registered patients and working effectively with all colleagues within the team to deliver high-quality patient-centered care.

This autonomous practitioner will be responsible for assessing, implementing, developing, and evaluating programs of care for patients with chronic diseases, minor acute illnesses, and contraception, in line with relevant knowledge and skills.

The successful candidate will provide nursing expertise and leadership to the wider primary health care team, ensuring that patients receive the best possible care.

Main Duties of the Job

  • Assessing, implementing, developing, and evaluating programs of care
  • Clinical risk assessments and formulations of management plans
  • Supervision and teaching for other staff
  • Immunizations and vaccinations
  • Assisting in developing local guidance and protocols
  • Clinical assessments, including history taking, and providing care for those with a range of illnesses/conditions

We provide inpatient and community-based mental health care for people living in Bolton, the city of Manchester, Salford, Trafford, and the borough of Wigan, and a wide range of specialist mental health and substance misuse services across Greater Manchester, the north west of England, and beyond.

The post holder will work as part of the prison nursing team delivering care to the general prison population and if required, the in-patient facility.

The team comprises mainly RGNs and RMNs with support from a wide variety of other specialist healthcare professionals.

  • Qualified RGN.
