Job Summary


The Advanced Primary Care Practitioner will be a key member of the Primary Care Team, providing comprehensive primary care services to registered patients. This role will work effectively with all colleagues within the team to deliver high-quality, patient-centred care.


This autonomous Practitioner will assess, implement, develop, and evaluate programmes of care for patients with chronic diseases, minor acute illnesses, and contraception, in line with relevant knowledge and skills.


Key responsibilities include providing nursing expertise and leadership to the wider primary healthcare team.


Main Duties



  • Assessing, implementing, developing, and evaluating programmes of care
  • Clinical risk assessments and formulations of management plans
  • Supervision and teaching for other staff
  • Immunisations and vaccinations
  • Assisting in developing local guidance and protocols
  • Clinical assessments, including history taking, and providing care for those with a range of illnesses/conditions

The post holder will work as part of the prison nursing team, delivering care to the general prison population and, if required, the in-patient facility.
