Job Summary

The Primary Care Specialist will be a key member of the Primary Care Team, providing comprehensive primary care services to registered patients. This role requires working effectively with all colleagues within the team to deliver high-quality patient-centered care.

An autonomous practitioner responsible for assessing, implementing, developing, and evaluating programs of care for patients with chronic diseases, minor acute illnesses, and contraception, in line with relevant knowledge and skills.

To provide nursing expertise and leadership to the wider primary healthcare team.

Main Duties of the Job

Assessing, implementing, developing, and evaluating programs of care.

Clinical risk assessments and formulations of management plans.

Supervision and teaching for other staff.

Immunizations and vaccinations.

Assist in developing local guidance and protocols.

Clinical assessments, including history taking, and providing care for those with a range of illnesses/conditions.

The post holder will work as part of the prison nursing team delivering care to the general prison population and if required, the in-patient facility.

The team comprises mainly RGNs and RMNs with support from a wide variety of other specialist healthcare professionals.
