Key Responsibilities
The Lead Community Pediatric Nurse is accountable for delivering specialized, proficient nursing care to children and young individuals with diverse health requirements, including acute short-term needs, long-term health conditions, complex needs, life-limiting conditions, palliative care, and ongoing care assessments.
Our goal is to facilitate timely discharges and avert unnecessary hospital readmissions for children with either acute or chronic health issues. This is achieved through the provision of specialized support and clinical expertise to assist in the assessment, treatment, and care planning, delivered by an independent practitioner. The post holder will serve as a resource of pediatric knowledge, offering specialized guidance to other professionals and promoting evidence-based practices.
Care will be administered in children and young people's residences, educational institutions, childcare facilities, and various community settings. The post holder will be expected to design, implement, and assess specialized nursing care programs. The role will also involve collaboration with partners across healthcare organizations, including primary care networks, to enhance an integrated pediatric nursing service.

Detailed Job Responsibilities
The Community Pediatric Nursing Team is dedicated to providing a high-quality in-reach and outreach nursing service that ensures safe hospital discharges and prevents avoidable admissions.
The team maintains strong connections with colleagues in our Specialist Nursing teams, addressing a variety of long-term health conditions, including Endocrine and Diabetes, Respiratory, Allergy, Neuromuscular, Bladder and Bowel, Complex Needs, Palliative Care, and Epilepsy.
We also collaborate closely with our colleagues in acute pediatric clinical settings and those from our Community and Neuro-developmental Pediatric teams, as well as primary and tertiary care professionals and educational staff.
Responsibilities will include, but are not limited to:
Playing a key role in the multifaceted care delivery processes and enhancing partnership working to ensure a seamless patient journey. Regularly developing, implementing, and evaluating individualized care programs. Supporting the management of integrated acute and community care pathways for children with both short and long-term conditions. Educating families and caregivers to perform specific complex nursing tasks, empowering them to confidently care for their child. Performing clinical skills such as blood collection, care of central lines/ports, and wound management. Administering medications, including intravenous therapies and cytotoxic drugs, in collaboration with pharmacy professionals. Maintaining awareness of current medications and therapies for pain management and other symptoms, requiring specialized knowledge in pediatric palliative care and symptom management. Handling sensitive and complex information with empathy and reassurance. Demonstrating exemplary clinical practice, staying updated with advancements in nursing care and treatments, ensuring evidence-based practice through current research. Actively participating in and promoting clinical supervision and reflective practice. Utilizing computer systems effectively and possessing strong IT skills.Qualifications and Experience
